Individual Information Sheet (Notice Individuelle)
Criminal Record Extract No. 2 (Bulletin n° 2 du casier judiciaire)
Curriculum Vitae (CV) - Declaration of Employment Periods (in particular if Inserm or SU)
Copy of diploma, translated into French/English if applicable
Copy of Health Insurance Card (Carte Vitale)
Copy of National Identity Card or Passport
IBAN/RIB
Declaration of Transportation Expenses (attach a copy of the Navigo card and the certificate)
Family Allowance Form (with associated obligatory documents)
EU citizens: Residence permit and work permit for foreign workers with a contract of more than three months: number, date of issue, duration, and validity.
Non-EU citizens: Passport and residence permit and temporary authorization for foreign workers with a contract of less than three months: number, date of issue, duration, and validity.
